Patient intake from first tap to the chart in four minutes

Accessible digital intake, consent, and eligibility workflows replaced clipboards across a multi-clinic provider network.

The situation

Front-desk staff re-keyed paper histories and insurance cards while patients repeated information already held elsewhere in the health system.

The constraint

The flow had to work on older phones, preserve consent provenance, protect PHI, and deliver structured data into different EHR configurations.

The approach

We designed the program around operational continuity and evidence. Each release had to prove it was at least as safe and understandable as the process it replaced.

1

Reduced questions by appointment type and prior record context.

2

Validated accessibility and completion with patients and front-desk staff.

3

Introduced clinics in cohorts while measuring chart delivery and correction rates.
